my game crashes when I equip the skin, I've tried limit adjuster, stream memory fix, made sure the name isn't the same as anything else and nothing worked

When I get killed by the sea monster at Fisher's Lagoon it triggers a cutscene that kills me and the cutscene is on repeat even after I respawn at the hospital.
